I'm looking for advice regarding applying to a specialty with less than 5 sites. For instance, psych and neuro have only 2 sites, respectively, but we need to rank 5 locations.

The question of what to do with the other 3 spots is complicated by the rumor I have heard that if you rank something like psych (that has more applicants than spots), followed by something like neuro (that has less applicants than spots), the Army may place you in neuro (to fill the program) even if both you and a psych program have ranked each other highly. Any thoughts on this?

Rank transitional years for the other spots, preferably at the locations where psychiatry (or whatever) has residencies. It is obvious to program directors which applicants aren't serious applicants, if you haven't interviewed, done an audition rotation etc you will be ranked at the very bottom of the rankings, regardless of the strength of your application. Frankly, you aren't wanted by the residency in that scenario.
