How would you rank Tx IM programs if youre thinking about fellowship ( not considering their location)?
UTSW
Baylor
San Antonio
Lubbock
UTMB
A&M
Houston
 
UTSW is the top program for medicine in TX and will carry the most weight for fellowship placement especially for other top tier programs. Baylor is probably the next best on your list even though many will say that Baylor is going through some rough times but that really shouldn't affect your training. UTH, UTSA, and S&W IMO are probably in the same level in terms of name recognition and training. I would stay away from UTMB just because of the post Ike situation. Texas Tech is the lowest ranking program on your list in terms of recognition and research opportunities.

In the end, if you don't mind staying in Texas for fellowship, then places like UTH and UTSA and S&W will provide you with good training and gives you the upper hand for staying in house. However, if you want to go to more big time programs or are interested in cards/GI, then UTSW is where you want to go.

Now I’m sure you all are wanting an explanation. Its obvious that UT Southwestern is king in Texas. I don’t think anyone would argue that. As far as fellowship placement I think Baylor has stumbled but the remaining faculty are still very highly respected and can place you in strong programs, especially anywhere you want in Texas. I was heavily warned not to go to Baylor, but I still think it deserves a lot of respect.


The debate will probably lie with the middle 3 programs.
#3: UT San Antonio is the next strongest due to the pathology and load the residents see. Well trained residents inevitably carry the reputation of a program and the residents leaving San Antonio are very well versed due to the autonomy they seem to be given. They have a very busy Tertiary Care Center/ County Hospital, one of the largest VA Hospitals in Texas and their referral center is the largest of the middle 3 programs. They also seem to be building new facilities incessantly at the moment which is a great sign in the current economy. Fellowship placement is strong here in Texas in all fields, with many each year matching all over the US. The only exception is if you want to stay in house for Cards. I was told this is recently due to the new Medicine Chair and Vice Chair, Dr. Hillis (fmr UTSW IM program director) and Dr. Lange (fmr JH Cards program director). They have only been taking Cards fellows from the "top 10" programs. Of course letters from two of the most prominent cardiologist can't help right.... Otherwise solid place. GI is also expanding from 2 to 3 spots soon.

#4: Texas A&M is also a solid place. It has some of the nicest facilities I have seen. They are situated in the middle of nowhere, but their referral base is several million due to its proximity to Austin and Waco. They do have a small VA, but only spend a month or so there a year. As far as fellowships, you are assured a spot in house and they have a surprisingly good record of placing people throughout Texas. Leaving the state is a tougher thing to pull from this institution, but anything can be done. If you can handle 5-6 years in Temple, then you'll have a pick of fellowships.

#5: UT Houston is situated in the Texas Medical Center, a place only rivaled by few.... but UT-H seems to struggle under even in the midst of such resources. They have a strong showing each year from FMGs and seemed to not be able to keep their own IM candidates. Their match list is mostly in house with few going elsewhere. Biggest strength they have is their tie with MD Anderson. While a loose tie, many residents do match their for HemOnc due to their ability to due electives there and get involved with Research. If you want to do HemOnc, then UT-H is much, much higher on this list. Cards has some access to Texas Heart, but Baylor pretty much owns this. I was told the mentorship here is lacking from faculty. Solid Medical School though.

#6-7: UTMB has fallen from grace due to unfortunate circumstances and Tech simply doesn’t carry the prestige to place you anywhere but in house. No GI fellows at Tech.

I threw in MGH bc I've read posts where people actually place UTSW in the same category as MGH. UTSW is a good place, but as Ochocinco would put it, "child please!"

More like MGH>>>UTSW>Baylor. I don't know what particularly you're looking at that makes you think Baylor's list is superior there, but I see UTSW as being stronger than Baylor in h/o, cards, and GI...and pulm/CC.

Yeah, I'm gonna agree with you. If you look at UTSW in the last few years they've sent people to Duke, Stanford, Cleveland Clinic, Vandy, NYU, Sinai, Wash U and obviously UTSW (a very strong program as well). The list that's linked above is only for 09- http://www8.utsouthwestern.edu/utsw/cda/dept26481/files/90666.html has the previous years. I'm just focused on cardiology so that's all that matters to me.

I was incredibly impressed with UTSW's match list and I'm a med student in the Northeast, miles away from Texas 😉 No doors will really be closed coming from there. I was very impressed just how good a reputation it has among docs even up here in New York.

They do quite well out of house. I've noticed programs who take IMGs do pretty well in terms of fellowship matches. They have great stats and tend to have a lot of publications. (Or at least the IMGs who match to programs the quality of UTSW tend to have a lot of pubs.)
 
I would say UTSW #1 and then Baylor. Would check into what is going on at Baylor recently, though, and try to judge how it will affect your training...I really don't know.
UT San Antonio has matched some people well in recent years.
