I was an Out of state applicant with absolutely zero tie to Missouri (Or Kansas city), but I was accepted last year. If you are OOS, and they still invite you for an interview, there is a pretty good chance you will get in. The school has a separate IS, neighbouring state, and OOS waitlist, which is an advantage for OOS interviewees.
A reason why there are not many OOS enrolled students here is probably because the OOS student who they accept do not go there, and those seats will be given to IS student. During my interview they told us, the earlier year they offered seats to 5 OOS students (out of their 20 seats) but only 1 came. The other 4 unfulfilled seats eventually went to either IS or neighbouring state applicant. If I were you, I would apply, because trying to save a little money in a grand scheme of things is not that important.
For people who might be wondering what the interview looks like, last year, the school interviewed only 2 days. Half of the applicants on the first date and another half on the 2nd date. I believe they interviewed 88 students with 44 each day. The date started with the information session, then the interview, then an optional tour of the hospital.
Interview is MMI format only. 44 students were divided to 5/6 groups (can't remember exactly how many). Each group were taken to different rooms for MMI. The room set up was not great. Only curtains were blocking a pair of interviewer/interviewee from another pair. So when all people started talking, it got loud. Be sure to speak loudly. There was no one-on-one interview.
The hospital experience at this school is great. The BS/MD students seemed very smart. OOS can also get IS tution after fulfilling some criteria, that is another plus point. Overall, I was impressed.
