I would send 1 evaluation form with the letters you are having sent via your premedical committee (I assume that is what you are doing). The pre-med committee will undoubtedly know how to handle the form (I don't know what it entails b/c I did not apply to S. Carolina). BTW, are you a resident of the state of SC? I notice that you are at UCLA, so unless you have exceptional stats or are MD/PhD (3.8+ GPA, 35+ MCAT) I would not apply if you are a non-resident or do not have strong ties to the state. U So. Carolina only takes a couple of out-of-staters each year, as does MUSC (in Charleston). good luck!
