Man I am not a good luck shadow lately.

At the surgeons, we had a diaphragmatic hernia repair, and pre-op I was talking with the intern I was with and he was quizzing me about the complications of the repair. We talked a good bit about re-expansion pulmonary edema, specifically. 6 hours later the dog dropped dead from re-expansion pulmonary edema.

Last Friday I was asking the ER vet about a jugular cutdown, because I saw it in a SOAP and have never heard of it before. He explained it, and said that luckily he’s never had to do one, because he’d be scared af that he’d mess it up.

2 hours after I left, he had a patient come in and he had to do a jugular cutdown.

There’s been more too! But like holy cow. I’m not superstitious but I am a little stitious.

You're not bad luck 🙂 Honestly, I've found ER things come in waves. First off, every time we look at a piece of equipment we never say "We haven't used that in a while" because of course then we need to use it that day! We never say the dreaded "Q" word or talk about the pace of the day 😛 but the waves thing is REAL!!!!! We'll have 4 things come in with high liver values, or 6 with diarrhea, or have the dreaded chihuahua wave where 5 chihuahuas come in!!! I think it's a lot of fun at the end of the day to see what kind of a wave it is 🙂 We always say, things come in at least 3s! I know it sucks to have things that are considered bad to happen to patients and you while shadowing but consider that it's best you get to see it while you're not in charge and everyone is under strict supervision!! It's the best way to learn 🙂

We had a cervical dorsal laminectomy yesterday that I scrubbed in to, and the surgeon pointed to a structure and said “you see this? What’s this?”

And I don’t know how, or why, or what, but my soul told me to say the nuchal ligament (in which I had completely forgotten that name).

AND I WAS RIGHT!

Taking 2ish weeks off from working starting next week before classes start again (other than some research stuff I'll have to keep up with), and honestly can't believe how fast this summer flew. It was great and definitely different compared to the last several summers I spent working in general practice. But it wholeheartedly confirmed for me that surgery is for me and I basically don't want to do anything else haha. Thankful for a really fantastic mentor who has made it all seem so possible and let me be so involved with so much.

You get time off? Lol I'm jealous. I'm just sitting here frantically trying to do things I have no business doing. And I should be writing that paper in my "down time". I'm a little stressed lol

I'm just taking the time off lol, I'm probably going to still end up at school a couple days, but I'm at least trying lol. I've been at it since the day school got out, that's my excuse anyway lol...when do you guys start again?

Because I am indecisive, but I know that I would be happy, but instead want to torture myself for 4 more years to be something else. :laugh:
Not to smack you but to smack you for wanting to do a residency when you don't need one! Also I'm probably a bad candidate for this job since I also want to torture myself for an extra four years :laugh:

You could probably make as much or more in your first year going straight to ER than you would in the four years doing an internship plus residency.

Does that convince you? 😛

Count me in for the voluntary torture, but I don't think I can hack an internship so I'm hoping to match into a residency out of private practice. On paper it can be done. But that's where the internships are going to be super important... my chances are very likely going to be better if they've met me in person and seen me do the things!

That’s the best I got because I’m in it for the torture long-haul lol.

But I’m glad you love ER.

Didn't you want to do ER for the emergency surgery aspect? You're not gonna get that as a criticalist 😛

I guess you have to decide if you like critical care more or surgery... because no surgery as a criticalist...you got time

It does matter! But that’s short term vs long term money! From my research, you would make more as a criticalist, so it’d be long term money vs the money you would earn not doing a residency!

Why not go out and work in ER and see if you feel like you want more of the criticalist aspect later?

Agreed. You can always go back for more training. (But it will be hard to go from that sweet ER money to residency money...)
 
I'll be there the week prior to help with orientation. So only 3 weeks left. 🙁
 
We start the 20th. Not emotionally prepared. Summer breaks sure have been nice.
 
1) We were almost on page 2. Unacceptable.

2) I just submitted my 4th year rough draft schedule to ASA!!! I have confirmed externships with 2 zoos and 1 large small animal hospital, and submitted my application for the zoo we partner with. I bribed ASA with doing Small Animal ER over Christmas if they give me the off blocks I requested cause I have 4 weddings to work around. 😆
