Northwestern Vs. Case

Both schools are great, you can't really lose here.

That being said, I'd go with NW. Chicago > Cleve, Big 10 > D3, and I'd give NW the edge in quality of home residency programs (though it's close... CCF is more "elite" in certain specialties + CRWU has Rainbow Babies).
 
I liked both a lot. They're both PBL so they're similar in that regard. But I think I like Case's curriculum a bit more because they're blocked and have the optional USMLE style tests to do after their essay tests or whatever. With that being said though, I probably would have picked Northwestern, I felt like I'd be happier there, though this may have been largely affected by Chicago>Cleveland. I think reputation-wise, here in Canada, educated people would have heard of Northwestern but fewer people would have heard of Case, if that's what you're into. But you can't really go wrong either way, as said above.
 
I didn't interview at either but I'd sure as hell rather live around Evanston/Chicago than ANYWHERE around Cleveland.
 
There's a reason LeBron left Cleveland, OP. Go to NW.
 
Where should i go? Do they have similar reputations?

Reputations are the same among residency programs. Case has a more diverse group of hospitals for you to choose from regarding rotations. Northwestern is in a better location.

Prioritize by money first, location second. You'll get an amazing education at either place. PM me with any specific questions you might have regarding Case.
 
What is the difference in COA? What did you like/not like about both?
