2014-2015 SUNY Downstate Application Thread

This forum made possible through the generous support of SDN members, donors, and sponsors. Thank you.
Thank you! But what does it mean by a "notarized" copy of your drivers license. And for the tax forms, what exactly is it asking for? Which forms lol

Lots of hoops indeed =\
Lmao, ahh the hassles.
So for notarized docs, I imagine you just find a notary public somewhere, bring a copy of your license and have them put their little stamp on it.
For the tax forms, I imagine it's a copy of your tax returns, both federal and state. So for NYS I would submit form IT 201, and Federal a 1040EZ. but apparently if your not emancipated then you submit your guardian's info.

I filed as independent though, as I am 25 years old. And what's a notary public? Lol

A Notary public is someone who was "appointed by the state to serve as an impartial witness" (they took an exam and swore they would uphold the law) to any documents that require a witness. It's really just another hoop they want you to go through to verify your docs.
You can just google notary publics close by you. I think UPS does it as well, but good luck finding the right person at the right time. Most of them require some $$. You may get lucky and find someone who will do it for free.

Also, I'm not 100% sure but I don't think emancipated is the same as filed taxes independently.

I emailed the admissions office to ask about the residency form and they said you only have to fill it if they request you fill it. But I still have the x mark. Anyone else have this problem?

Just to be clear, accepting a spot on the alternate list won't jeopardize your acceptance to another school, right?

Btw, anyone know if we can send the health forms in via email?

I don't think they are checking boxes for items you didn't fill out. I emailed them the same question and they just linked me to the form, so I assumed I should go ahead and fill it out (I'm in state).

yep, i scanned and emailed in my health forms and lab slips at around midnight, and they were all approved by 10am the next morning. Student Health office doin werk.

Thanks I'll go ahead and email them for it then.

i'm also confused about the financial literacy if anyone actually gets it

the link for the financial literacy posted on my checklist does not work. I searched for the website and need to enter an access code: anyone know what it is?

Anyone know if we can just scan and email them our completed health forms? Or do we have to mail it in?

I finished the Admissions Study Styles Inventory yesterday, but it still hasn't been checked off on the check list. did anyone experience the same thing?

Yup, I completed it almost a week ago and it still hasn't been checked off.

To those accepted off of the alternate list - how long did it take for you to get the acceptance email? I received the call a few days ago, but haven't received my email.

So when you say you emailed them in do you mean to the normal medadmisions email address or to the student health email address?

If an alternate lister doesn't complete all the required things on the checklist by July 1st does it hinder the possibility of getting accepted off the list??

I got accepted off the alternate list Thursday and got the email Friday

Does anyone know if the transcripts must be sent to Downstate directly from the school or can I request a sealed envelope be sent to me and then I email it to Downstate?

quick question - i'm an OOS student. can you get IS tuition for MS2,3,4 after living off campus starting MS1?

Yes you can. Get your name on your lease, have bills sent to you, change your license & voter registration to NY, and you'll be set.

CAN SOMEONE PLEASE POST THE AAMC FINANCIAL LITERACY LINK. NONE OF THE LINKS ARE AVAILABLE ON MY CHECKLIST. THANK YOU

Is the sign-in only for accepted students or alternates as well?

Have any alternates received confirmation of submitted materials?

Only the alternate form has a check for me despite sending other stuff last week. The acceptance response form also has an X 🙁

I only received a check for the alternate list response form after over a week, and then me emailing them, and then me calling them a few days after that. I did the study styles survey a while back and never received any check for that. I wouldn't worry too much about it.

I sent an update around two weeks ago but never received a confirmation for it even though I know they usually send one, is this something I should be concerned about? I'm worried that maybe it was missed or something did anyone else have this problem

Does everyone on the alternate list has 'Acceptance Response Form' on their checklist?

I received confirmation for all my health forms, but not for any other forms at the top of the checklist besides the alternate response form.

Where does the sign in and password come from? Is it the same as the secondary application?

Does the admin office sent it to you??

I received two emails from Admissions with my username and password info on May 29th.

How long did it take for the people accepted off the alternate list to get their official acceptance letters in the mail?

Update: Rejected 6/5. 4 months after interview with no communication in between except me pestering them!! But accepted at another school yesterday. Good luck to everyone still waiting - hang in there.
