It seems to me that you send that to your schools Office of Student Conduct along with your statement (I'm assuming this is similar what you wrote for AMCAS) and request that your schools official complete and submit the form & statement to MCW. I would email MCW admissions if you are unsure though!

I would say this is true for most (if not all) medical schools! Just because someone who submitted later than you gets an interview invite doesn't mean an automatic rejection. It's still super early 🙂
