I shall spell this out in detail to hopefully calm you people down.

This is how it has worked for the past 4 years, at least:
MD: The admissions office decides on a certain Saturday every month-ish to send out acceptances. If your interviewer feels particularly nice or has some downtime, they
may call an accepted student on the Thursday or Friday prior to the decided upon Saturday. All accepted students (for that round) will have a status change at 12:00 AM on Friday night/Saturday morning (even those who already got a phone call).
If you don't hear anything all that means is that you haven't been accepted yet. For MD, WashU does not reject or waitlist until the spring. They reconsider all interviewed students for each acceptance round so you can interview in October and get accepted in January.
MSTP: While at the interview, the MSTP people tell you a Friday on which you will hear a decision (accepted, waitlisted, rejected). You will hear then... easy enough. This is not coincidentally the day before the acceptances go out for the MD students.
I highly doubt this has changed much. IIRC, they have always mailed something to accepted students, the status change just comes first.
For reference the status change was to something like "Congratulations. Pay us $XX to hold your spot in the class." on the main page of the WUMSApply thing.
Keep in mind that this could have changed from last year, but our admissions process is pretty set, so I doubt big changes have happened. If so, hopefully someone from the adcom will read my false info and email me how it's actually working so I don't stress you people out more than you already do yourselves.
